To request to change your membership category at any time, please complete and submit the below form.

Please note that all requests to transfer into another playing category takes into consideration the playing vacancies of that category at the time of your request. Should there be no vacancies in that category you will be placed on a waitlist until a position becomes available. Vacant positions will be offered to the waitlist in the order that requests are received.

  • Please attach any supporting documents. NOTE: If you are requesting a Medical Leave of Absence, a medical certificate must be attached

  • Please note: Requests cannot be backdated and changes take effect from the 1st of the month.
